Most of us think if we don’t have pain in our teeth, they are healthy.  But this is not true. Most of the dental problems are asymptomatic in starting and can be diagnosed only by routine checkups. If symptoms start then you are in a big problem!! 

Same happened with her 

It all started last week when it was raining outside. 

I still remember ‘What a beautiful evening it was!! 

When a beautiful young lady who in her 30’s entered our clinic with tensed face, and trembling lips. Slowly, she said “my jaw hurts, I would like to see the doctor. 

Our team made her comfortable and arranged her check-up in room 1.  After doing a thorough check-up, I noticed she had a missing tooth in her lower jaw. I told her that her teeth arrangement is the root cause of her pain.  

While discussing her case when I asked how long it has been that she has lost the tooth. She said it was a long time ago, like 4-5 years back because the tooth was decayed and half-broken so she had to get it removed. 

Then I asked her “why you didn’t get it replaced at that time? She replied “it was just one tooth Dr and I was chewing fine so I thought it is not necessary to replace it. 

I said “that is the root cause of the problem. Tooth always survives in pairs.  When we loose one tooth multiple things can happen. Opposite teeth might start growing to fill the space, adjacent teeth might start to drift to fill the space leading to abnormal contacts and our bite tries to adjust it.  

Not everyone develops the same kind of symptoms 

Some might develop pain like you, others might develop cavities or  gum problems due to abnormal contacts, a few might develop jaw joint  problems, or maybe all teeth start getting abraded

One day a 25-year-old male patient visited our practice with the complaint of moderate pain in his left lower back teeth. 

After doing his oral examination and X-rays, we explained to him that his left lower wisdom tooth was the one causing pain. 

Listening to this he said- ‘’Doctor what are wisdom teeth, as I have heard a lot of my friends getting wisdom tooth troubles? What problem is it causing me and What all other problems can wisdom teeth cause? 

Although a common problem still we see that there is a lot of anxiety around wisdom teeth and their removal in the minds of people. To ease him we explained both verbally and through videos about wisdom teeth and problems with wisdom teeth. 

 

Starting with- what are wisdom teeth? 

Wisdom teeth are a person’s third set of molars and the last teeth to develop and appear in the mouth at the age of 17 – 25 years, also known as the “Age of Wisdom” – that’s why they are called wisdom teeth. There are normally four wisdom teeth – two in the upper jaw and two in the lower jaw – but some people have fewer or even none at all. 

Answering his leading question- what problem was it causing him? 

We showed him on the intra-oral camera that his gums around the wisdom tooth were inflamed because that area was hard to clean and food was getting stuck between teeth and gums causing decay (cavity) in the tooth and infection in the gums. 

Now to answer his question what other problems can wisdom teeth cause? 

Some wisdom teeth come through without causing any issues, but others are problematic and may need removal or other treatment. 

Wisdom teeth can cause various dental problems including the overcrowding of the existing teeth, and impaction. 

Impaction is when a wisdom tooth comes through at an angle and pushes into the gum or the tooth beside it. 

Problem wisdom teeth can also cause infection, leading to pain, difficulty chewing and bad breath. In certain cases, wisdom teeth can lead to serious infections and swellings. 

 

Symptoms of wisdom teeth infection.

Signs of gum infection caused by wisdom teeth include: 

  • Red, inflamed gum near the wisdom tooth 
  • Swelling
  • Pain 
  • Pus coming from the gum 
  • Swollen and sore lymph nodes underneath the jaw 
  • Difficulty opening the mouth and swallowing 
  • Fever 
  • Bad breath

 

After knowing this he wanted to know what is the solution? 

He was explained- If wisdom teeth are causing any of these problems, it is recommended that one have them removed or extracted. 

He got nervous and asked, “is there any other option doctor?? 

Infections caused by wisdom teeth can be treated: 

  • By improving oral hygiene in the area 
  • By having the area cleaned by us 
  • Sometimes by the prescription of antibiotics. 

However, the infection may keep coming back if the problem tooth is not removed. 

So it is recommended to extract the wisdom tooth and for the procedure, you will be given a local anaesthetic so you won’t feel any pain– this is the most common pain relief option. 

We asked him to take his time to think about it and then let us know so that we can plan the extraction whenever he was ready. We reminded him of one more important thing – to keep a check on other wisdom teeth also so that you do not land up in similar condition for other wisdom teeth like you are in right now, because wisdom teeth are very unpredictable, occasionally it will erupt properly without causing any discomfort and sometimes it will cause severe pain that you will not be able to bear. 

For avoiding such problems, it’s recommended to follow these steps: 

  • Brushing and flossing twice a day 
  • If you are not able to floss in between the wisdom tooth you can use a water flosser that is easy to use and very effective. 
  • Visit a dental clinic every 6months to allow your dentist to evaluate your wisdom teeth and your overall dental health.

Once in the evening, I was walking in my society garden and a lady approached me and said ‘’ Hi, doctor do you recognize me?

As I was trying to recall her name, she said ‘’I visited your practice 2-3 years back and I have to thank you, as because of you I am pain-free and my fear of dental treatment is also gone.

The lady’s words made me happy and who will not after getting an appreciation from a satisfied patient.

I said, “I am glad you are happy with my treatment.”

Then she said “sorry for bothering you doctor in the middle of your walk, if you have time, can you please tell me one thing how can I maintain and brush my teeth so that I only have to visit you for my routine check-up. I remember you explained the process to me during my treatment, however, it slipped out of my mind. Can you please explain that to me again?”

It’s always good to see people concerned about their oral health and hygiene. Our teeth are to be treasured. They are much like every other part of our body; they can’t be easily replaced. With the strain they get from all the chewing, biting or grinding, they will also wear and tear if one doesn’t take care of them from time to time.

I started by saying that — the first and most important measure to make your teeth healthy is to brush twice a day and floss daily, change your toothbrush every three months, use a medium bristle toothbrush, avoid very refined foods and sodas, eat a well-balanced diet, rinse your mouth after every meal, use toothpaste that has ingredients like hydroxyapatite, massage your gums with a pad of index finger at least once a day.

She further asked, “But Doctor, I don’t know how to floss. The one time I tried doing that, I failed to remove the food particles stuck in between my teeth which lead to bleeding of my gums.

I replied, “Don’t worry Ma’am, you are not the only one. Most people face the same problem as you do too. Many interdental aids are available in the market which is easy to use and economical as well. For instance, you can use interdental brushes, thread floss or a water flosser as well. 

If thread floss is difficult for you to use, try using a water flosser which is very convenient to use and not very expensive. It comes with two/ four tips that can be used by you and other family members, these tips are color-coded so it’s easy to remember. You know what, I am a big fan of water flosser because it is hassle-free, more effective and different from other interdental cleaning aids. 

If you follow all these instructions then you don’t need to see us in pain or with any other problem related to your teeth, you have to visit us only for your routine check-up.

She said, “ I will definitely follow all these instructions and also make it a routine planned for my family follows this. Thank you Doctor for giving me your time and explaining all this in such an easy way. I replied, “I will be much happier if you visit me only for your routine check-up. It will make me feel like I was successful in making you follow these instructions as needed. She smiled and said that she would try her best not to let me down on these grounds. We smiled at each other and resumed our walk together.

Regular dental cleanings are an essential part of good oral hygiene. After being in the field of Dentistry for more than 12 years, we have had many experiences of patients having a lot of queries and doubts about teeth cleaning. This is one of the recent experiences we have had in the past.

One day, a young lady came to visit the clinic with a complaint of bleeding while brushing her teeth. Soon after attending to my scheduled patient, I, Dr. Garima Malhotra went to check this lady patient, who was looking quite anxious and nervous. Immediately after sitting on the dental chair, she questioned nervously if I was about to perform a painful procedure. I smiled and told her to calm down. I assured her that I was only getting her check-up done. On seeing her anxious, I asked her if she had had any past dental treatments that had ended up bad. She said, “no, I have heard that dental treatments are very painful.” I tried to explain that back in the old days’ dental treatments were very painful, however, now improvised technology has helped us in making dental procedures pain-free. After listening to this, she felt very comfortable and trusted me that I wouldn’t cause her any pain when examining her. Following her symptoms, I examined her and explained to her that she needed cleaning of teeth and proper gum care as she had a lot of deposits and plaque around her teeth which caused the gums to bleed while brushing. Anxious; she asked me if cleaning would make her teeth weak and loose. After listening to her I ‘smiled’. She asked what happened Doctor? Then I said ‘you know most of the people are living with this myth’. 

I explained to her with the help of an example; water contains many minerals and one of them is calcium that you see around your bucket when you store the water for a few days in a container or bucket. Just like that, saliva in our mouth also contains calcium that gets deposited around our teeth and this deposition grows at the cost of your gums. It leads to the separation of your gums from the teeth and forms a band around them. These bands are most commonly found in the anterior part of the front lower teeth and the exterior part of the upper back teeth. All these deposits usually don’t cause any symptoms hence go unnoticed. 

Very few patients develop symptoms like bad breath, bleeding from gums while brushing and in worst cases, these depositions form a thick band around teeth and act as an anchorage. Hence, when cleaning is performed, patients feel like their teeth are about to fall off or feel loose, however, after removing the deposition, the gums start to heal and become firm and healthy. After listening to the explanation she asked, “so doctor how can I avoid these depositions from building on my teeth and what is the right age of cleaning? 

Do I need to get my teeth cleaned every month or every alternate month?  

For clearing her confusion I explained “See it’s not like that you need cleaning every month because every person has a different deposition rate so some patients need cleaning once a year, some in every 6months and some patients don’t need it at all because their deposition rate is very low they only need to do routine hygiene measures like brushing and flossing. Now, to answer the recommended age of cleaning; the right age for getting a cleaning starts from young. If you start your cleaning at a young age we can control the deposition at its early stage to know the frequency rate of your deposition so that we can guide you when to visit us, also teach you how to maintain your oral hygiene. So, if you follow as told you can live with your natural teeth for a very long period of time or till the end of your life. After this conversation, she was pleased and agreed to do regular cleaning in order to strengthen her gums and maintain her oral hygiene.
