- Five Consultations with Principal Dentist

- Yearly Oral Prophylaxis (teeth cleaning) as many Appointments required
- Yearly Polishing of teeth
The 999 plan requires a minimum purchase period of two years.

- Unlimited Consultations with Principal Dentist
- Priority Consultations with a Specialist
- Digital X-rays as needed
- Bi annually Oral Prophylaxis (teeth cleaning) as many Appointments required
- Bi annually Polishing of teeth
- Fluoride Treatment if needed
- 10% Discount on all Treatment except Implant and Braces
- 5% Discount on Implants and Braces
- 5% of Treatment Vouchers on Referring Friend or Family Members
Our Membership Program is not insurance. The program is offered to individuals and families and is not transferable to another dental practice.

Membership is valid for 1 year from the enrollment date. - Membership dues are payable in full upon enrollment and are non-refundable. No refunds or premiums will be issued at any time if participant decides not to utilize their dental membership
- Cash, check or credit card payments are accepted.
- Membership will automatically renew on the anniversary date unless a written request to cancel is received 30 days before the renewal.
- Membership must be current to receive the discount.
- Fees for dental services may change at any time.
- It is the member’s responsibility to schedule and keep all appointments offered as part of the Dental Membership Program.
- Please notify our office at least 24 hours in advance if you must change a scheduled appointment.
